Dear Program Director:
The 16th Annual Scientific Forum of the National EpiFellows Foundation (NEF) will be held at the Pan Pacific Hotel in Seattle, Washington on December 4-5, 2008 (one day prior to the American Epilepsy Society 62nd Annual Meeting). The NEF Scientific Forum is supported by Pfizer Inc.
You are invited to nominate an epilepsy fellow from your institution to attend this NEF Scientific Forum. Registration is limited to the first 50 nominees, and registrants will be selected on a first-come, first-served basis (priority will be given to first-time attendees). This will be our third year, where 15 fellows from international programs are being invited. Nominees should be neurology trainees in approved epilepsy fellowships; in rare instances, current neurology residents with a well-defined interest in epilepsy will be considered.
Topics presented at the Forum will include: Post Traumatic Epilepsy – Prevention and Treatment, Cognition and Cognitive Abnormalities in Epilepsy, Mechanisms & Treatments in Neonatal Seizures, Neurodevelopmental Effects of AED Exposure During Pregnancy, Epilepsy Guidelines: the Good, the Bad, and the Ugly, and Multimodal Outcomes of Epilepsy Surgery.
The selected nominees will receive the following from the NEF:

HOTEL ACCOMMODATIONS
(for US Fellows) a 1 night accommodation at the Pan Pacific Hotel in Seattle, Washington on Thursday, December 4, 2008. Additional hotel nights at the group rate can be arranged, if available, at the registrant’s expense. |

(for International Fellows) a 2 night accommodation at the Pan Pacific Hotel in Seattle, Washington on Thursday, December 4 and Friday, December 5, 2008. Additional hotel nights at the group rate can be arranged, if available, at the registrant’s expense. |

MEAL FUNCTIONS SPONSORED BY THE NEF specifically, the reception on Thursday evening and breakfast and lunch on Friday. All additional meals, either en route or for the duration of the AES Annual Meeting, will be at the registrant's expense. |

REIMBURSEMENT OF EXPENSES only travel expenses incurred by the registrant for air transportation, or mileage for those registrants driving to the meeting. Expenses to/from the registrant’s home to/from the airport, and/or alternative means of transportation used when transfers have been provided, will not be covered. All other instances will be determined on a case-by-case basis. |

NOMINATION FORM
Please complete the nomination form and return it via e-mail or fax to Elena Spinelli (contact information below). Nominations will be limited to 1 fellow from each institution, with first-time attendees given priority.
REGISTRATION FORM
The nominee should complete the registration form and return it via e-mail or fax to Elena Spinelli by September 26, 2008. A full agenda will be sent to registered attendees as soon as it is available. Please note that the nominee’s signature on the registration form constitutes a binding contract. (See the disclosure section of the registration form.)
CASE PRESENTATION SUBMITTAL FORM
All attendees will be required to submit a case study with their registration form for possible presentation during the afternoon workshops. The nominee should complete the case presentation submittal form and return it via e-mail or fax to Elena Spinelli.
